The physical exam and medical history forms are required to be on file with the athletic trainer in order to participate in Westwood Athletics, including try-outs and off season. You may hand in the form in person to the Athletic Training Room (A1112) located in the fieldhouse, or send via email to susan_slagle@roundrockisd.org or john_horsley@roundrockisd.org.
