Athletic Forms
Pre-Participation Physical Exam Forms:
The physical exam and medical history forms (below) are required to be on file with the athletic trainer in order to participate in Westwood Athletics, including try-outs and off season. You may hand in the form in person to the Athletic Training Room (A1112) located in the fieldhouse, or send via email to susan_slagle@roundrockisd.org or john_horsley@roundrockisd.org.

Doctor Visit Form:
Please print and take the Physician Referral Form when visiting your doctor for any illness/injury/medical condition.
Physician Referral Form - Westwood High School ALL Doctor's visits require a note from the doctor brought back to the Westwood Athletic Trainer to show your restrictions or if you are clear to participate. This includes but is not exclusive to family doctors, general practictionors, dentist, dermatologist, psychiatrist, orthopedic, pediatrician, etc...
Concussion Management Forms:
Both Concussion Management Forms must be filled out and returned to the Westwood Athletic Trainer prior to beginning an athlete's Concussion Return-to-Play Protocol.
